Family rates: 10% off each person each month
Membership pricing
One time enrollment fee of $100.
The enrollment fee will be WAIVED for those who sign up during pre-enrollment. To sign up and save your spot during this pre-enrollment time, simply fill out the form on the Contact page. No payment will be collected until your first visit.

Primary care
$250 for the first office visit
$150 for subsequent follow-up office visits
Sports Medicine
$300 for the first office visit
$150 for follow-up visits (in the office or virtual visits)
$100 for corticosteroid injection + office visit fee
$80 for trigger point injections + office visit fee
$100 for Sports Physicial (preparticipation exam)
